Following are the guidelines to accept nominations and to screen the same. Each nomination shall be considered complete and accepted only if it adheres to the provided guidelines:
1. Nominations can be made across all categories and multiple nominations can be made in a category. A separate form must be used for each nomination
2. Self - nomination is not allowed. Nomination by third party individual/organization is allowed.
3. Winners of last year’s awards cannot apply this year.
4. The individual/organization should have initiatives contributing to the composite State of Andhra Pradesh or Telangana. (existing prior to 2.6.2014)
5. *All sections in the form must be complete, else it may not be considered as a valid entry. In case any field is not applicable, please write N/A.
6. Nominators can provide up-to 1 supporting file to demonstrate the impact of the initiative/innovation being entered.
7. The last day for submission of nomination forms is 30th April, 2024 at 6 pm.
8. Please refer to the Rules & Regulations document for additional guidelines on participation.
9. Participation in the Awards will be construed as an acceptance of the Rules and Regulations stated herein.

11. The determination of who should receive an award for any award category rests with the Jury and their decision is final and
binding on all applicants.
12. Information provided in this form will be kept confidential and will be used only for the purpose of evaluating this entry for the
Awards.
13. Awards and recognitions is for the period 1st April 2023 to 31st March 2024.
14. Only individuals with age between 18 years to 40 years as on 1st April 2023 are eligible for Young Achiever Awards.
15. Once the nomination is submitted, you will receive a registration Id for any future references. You can edit the nomination form with the Login Id & Password which is the Registration ID.
